The Employee Retirement System of Texas stated that a coding error on its password-protected portal potentially compromised the personal health information of 1.25 million people, HealthITSecurity reports. In a statement, the ERS said information included “first and last names, Social Security numbers, and ERS member identification numbers.” Meanwhile, North Carolina–based Catawba Valley Medical Center announced 20,000 individuals may be involved in a data breach after three employees suffered phishing attacks that exposed patient health data, and the Children’s Hospital of Philadelphia reported an email-hacking incident that put PHI of 5,368 patients at risk.
