The U.S. Department of Health and Human Services' Office for Civil Rights announced a $2.3 million fine against management company CHSPSC for violations stemming from a 2014 data breach that affected 6.1 million individuals. An OCR investigation revealed CHSPSC failed to respond to cybersecurity warnings from the Federal Bureau of Investigation about the breach, while also finding longstanding violations of the Health Insurance Portability and Accountability Act Security Rule.
