The Hamilton Paramedic Service in Hamilton, Ontario, discovered it mistakenly sent patient medical information to the wrong hospitals over a four-year period, CHCH News reports. The issue was first discovered by an out-of-city hospital in October 2023. A city audit of 306,000 ambulance transport records found 162 patients were impacted and none of their medical information was leaked publicly.
