A spokesman for a sexual health clinic in London says human error was the cause when the clinic mistakenly revealed the identity of almost 800 patients who’d “attended HIV services,” The Irish Times reports. Due to an administrative error, an email intended for individual recipients was sent to a group email, the report states. “We immediately contacted all the email recipients to inform them of the error and apologize,” said the spokesman. Meanwhile, a glitch in news retailer WHSmith’s contact form allowed for at least 22 subscribers’ personal information to be compromised, The Guardian reports. “Information typed into the form, which is supposed to then be passed on to the company itself, was instead apparently sent to its entire mailing list,” the report states.
